Form Strategy Guide

How Many Fields Should an Intake Form Have?

There is no universal correct field count. High-converting service intake forms focus on field relevance rather than arbitrary brevity.

The Rule of Immediate Decision Utility

Every field in an initial intake form should satisfy at least one of five criteria:

  • 1. Identify visitor type (prospect vs existing client)
  • 2. Qualify mutual fit & project scope
  • 3. Route to correct internal owner or team
  • 4. Select appropriate calendar type & duration
  • 5. Prepare the advisor for a productive initial consultation

Overbuilt Initial Forms

Asking for tax IDs, uploaded prior filings, full medical history, or legal contract history on initial contact causes up to 60% abandonment before mutual fit is even known.

Underbuilt Initial Forms

Asking only for Name and Email requires staff to make multiple callbacks or manual email exchanges just to learn basic service requirements before scheduling.

Progressive Disclosure Framework

Move secondary onboarding questions to a post-booking step or pre-meeting confirmation page. Once an appointment is secured on the calendar, completion rates for secondary preparation questions exceed 90%.

Intake Review

Want us to look at your current intake?

Book an Intake Review. We will look at how information is collected, qualified, routed, booked, and handed off today and whether a cleaner system makes sense.
